ABSTRACT Purpose: Concomitant nasolacrimal duct obstruction can occur in cataract carriers, which increases the risk of postoperative endophthalmitis. The primary aim of this study is to evaluate the knowledge of Brazilian cataract surgeons on the diagnosis and management of cataracts associated with nasolacrimal duct obstruction. Methods: This survey was based on a questionnaire involving Brazilian cataract surgeons that was conducted from March to April 2018. Data were collected on the participant's profile, time and experience in ophthalmic practice, previous training in diagnosis and management of nasolacrimal duct obstruction, and background with endophthalmitis after cataract surgery in patients with nasolacrimal duct obstruction. All data were entered into an Excel spreadsheet and analyzed according to the frequency of occurrence. Results: Ninety-one ophthalmologists answered the questionnaire. Most (63.7%) had been performing cataract surgery for >10 years, and most (84.6%) received training to diagnose and handle nasolacrimal duct obstruction during their medical residence training. Nasolacrimal duct obstruction was investigated in the preoperative period of the cataract by lacrimal sac expression test (53.8%) or by irrigation of the tear pathways (23.1%). Nasolacrimal duct obstruction was treated with antibiotic eye drops by 47.2% of respondents. Seventy-eight percent of surgeons indicate usually performing lacrimal surgery prior to the intraocular surgery, waiting for 4 to 6 weeks to proceed with the cataract surgery. The procedure of choice for treating nasolacrimal duct obstruction prior to cataract surgery was dacryocystorhinostomy (88.4%). Most participants recognized the need for a protocol to assist in the detection and management of nasolacrimal duct obstruction in cataract carriers. Conclusion: Improvement in the diagnosis and management of nasolacrimal duct obstruction concomitant to cataract is needed, as this is a risk factor for endophthalmitis.


RESUMO Objetivo: Portadores de catarata podem apresentar concomitantemente obstrução do ducto lacrimo-nasal (DLN), com risco de desenvolver endoftalmite no pós-operatório da facectomia. O objetivo do presente estudo é apresentar as percepções dos cirurgiões de catarata sobre a propedêutica e a conduta frente a pacientes com obstrução do ducto lacrimo-nasal concomitante com catarata. Métodos: Trata-se de uma pesquisa baseada em um questionário envolvendo cirurgiões brasileiros de catarata, realizado no período de março a abril de 2018. Foram levantados dados sobre o perfil dos participantes, o tempo e a experiencia da prática oftalmológica, o treinamento prévio para diagnóstico e tratamento da obstrução do ducto lacrimo-nasal e os conhecimentos de endoftalmite após cirurgia de catarata. Todos os dados foram inseridos em planilha Excel e analisados de acordo com a frequência de ocorrência. Resultados: Noventa e um oftalmologistas responderam ao questionário. A maioria (63,7%) deles realiza cirurgias de catarata há mais de 10 anos e a maioria (84,6%) recebeu treinamento para diagnóstico e tratamento da obstrução do ducto lacrimo-nasal durante o curso de residência médica. A pesquisa da obstrução crônica do ducto lacrimo-nasal no pré-operatório da catarata é feita pelo teste do refluxo de secreção pelos pontos lacrimais (53,8%) ou por irrigação das vias lacrimais (23,1%). A obstrução do ducto lacrimo-nasal é tratada com colírios antibióticos por 47,2% dos respondentes. Para os portadores de obstrução do ducto lacrimo-nasal , 78% indicam a desobstrução das vias lacrimais previamente à facectoma, aguardando de 4 a 6 semanas para tal. O procedimento de escolha para tratar a obstrução do ducto lacrimo-nasal antes da facectomia é a dacriocistorrinostomia (88,4%). A necessidade de um protocolo para auxiliar na detecção e tratamento da obstrução do ducto lacrimo-nasal em portadores de catarata é reconhecida pela maioria dos participantes deste estudo. Conclusão: É necessário melhorar a propedêutica e o manejo da catarata em portador de obstrução do ducto lacrimo-nasal porque esse é um fator de risco para endoftalmite.

ABSTRACT Purpose: To determine the reliability of the endoscopic dye transit test for the prediction of functional success after dacryocystorhinostomy. Methods: A cross-sectional study was conducted with 50 patients who underwent external dacryocystorhinostomy Group or transcanalicular dacryocystorhinostomy Group and had anatomically patent ducts during irrigation, with a minimum 6-month follow-up. The external dacryocystorhinostomy, defined as the time from instillation of the dye into the conjunctival sac until its flow from the rhinostomy site, was performed in all patients. Positive predictive value of the endoscopic dye transit test to assess functional success was analyzed. The cutoff point was determined using a receiver operating characteristic curve. Results: Of the 50 patients, 44 (88%) exhibited subjective improvement or complete resolution of epiphora (functional success). The best cutoff point for the endoscopic dye transit test was 60 s. Of 39 patients with endoscopic dye transit test £60 s, 38 (97.4%) exhibited functional success, demonstrating a 97.4% positive predictive value. Conclusion: The endoscopic dye transit test £60 s is a reliable tool to predict functional success and good prognosis after external or laser transcanalicular dacryocystorhinostomy.


RESUMO Objetivo: Determinar a confiabilidade do teste endoscópico do corante na predição do sucesso funcional após dacriocistorrinostomia. Métodos: Estudo transversal com 50 pacientes submetidos ao grupo de dacriocistorrinostomia externa ou grupo dacriocistorrinostomia transcanalicular e que possuíam dutos anatomicamente patentes pela irrigação, com seguimento mínimo de 6 meses. A dacriocistorrinostomia externa, definida como o tempo desde a instilação do corante no saco conjuntival até o fluxo do local da rinostomia, foi realizada em todos os pacientes. O valor preditivo positivo do teste endoscópico do corante para avaliar o sucesso funcional foi analisado. O ponto de corte foi determinado usando uma curva característica de operação do receptor. Resultados: Dos 50 pacientes, 44 (88%) apresentaram melhora subjetiva ou resolução completa da epífora (sucesso funcional). O melhor ponto de corte para o teste endoscópico do corante foi de 60 s. Dos 39 pacientes com teste endoscópico do corante £60 s, 38 (97,4%) apresentaram sucesso funcional, demonstrando um valor preditivo positivo de 97,4%. Conclusão: O teste en­doscópico do corante £60 s é uma ferramenta confiável para predizer o sucesso funcional e o bom prognóstico após dacriocistorrinostomia transcanalicular externa ou a laser.

ABSTRACT

Objetivo: comparar la técnica de sondaje de vías lagrimales previa dilatación canalicular con gel viscoelástico con el sondaje directo. Métodos: se realizó un estudio analítico prospectivo de cohorte no concurrente en 136 ojos pertenecientes a pacientes con obstrucción congénita del conducto lacrimonasal atendidos en la Consulta de Oftalmopediatría del Hospital Pediátrico Docente Pepe Portilla, de Pinar del Río, a quienes se les realizó sondaje de vía lagrimal, desde enero del año 2008 a julio de 2013. Se conformaron dos grupos de estudio, ambos de 68 ojos, uno con la técnica quirúrgica de sondaje directo, y un segundo grupo donde se aplicó dilatación previa de la vía excretora con gel viscoelástico. Resultados: en el primer grupo se presentaron complicaciones en el 45,59 por ciento. Se logró el 54,41 por ciento de curación y fue reintervenido el 32,35 por ciento. En el segundo grupo se logró el 97,92 por ciento de curación sin complicaciones incluyendo a 6 ojos de pacientes mayores de 3 años, y fue necesario reintervenir a un solo paciente. Conclusiones: el sondaje del conducto lacrimonasal previa dilatación con Healon minimiza las complicaciones y la necesidad de reintervención, y posibilita su empleo efectivo en pacientes mayores de 3 años(AU)


Objective: to compare the lachrymal duct probing technique after canalicular dilation canalicular with viscoelastic gel or with direct probing. Methods: prospective, analytical and non-concurrent cohort study carried out in 136 eyes from patients with congenital nasolachrymal duct obstruction, who were seen at the ophthalmological pediatric service in Pepe Portilla teaching pediatric hospital located in Pinar del Rio and who underwent lachrymal duct probing in the period of January, 2008 through July,2013. Two study groups were formed with 68 eyes each, one treated with direct probing, and the other with previous dilation of the excretory duct with viscoelastic gel. Results: the first group showed some complications in 45,59 percent of patients; 54,41 percent recovered and 32,35 percent were reoperated. In the second group, 97,92 percent managed to recover without complications, including 6 eyes from patients older than 3 years. It was necessary to re-operate a patient. Conclusions: nasolachrymal duct probing after dilation with Healon minimizes complications and need for reoperation, thus facilitating its use in patients aged over 3 years(AU)

ABSTRACT

ABSTRACT Obstructions in the lacrimal pathways quite often require accurate and reliable image scanning for confirmation and documentation. Infrared thermal imaging, known as thermography, is a resource that complements diagnosis; it does not require touching the patient or applying contrast materials and has been used in various medical procedures for decades. However, there have been few studies in the literature about its use in ophthalmology. In this paper, the authors have presented a case of dacryocystitis where the obstruction of the lacrimal punctum was so acute that conventional dacryocystography could not be used. The authors have successfully reported the use of thermography as a complementary propaedeutic and will discuss the method they used.


RESUMO As obstruções de vias lacrimais muitas vezes necessitam de exames de imagem precisos e confiáveis para confirmação e documentação diagnóstica. A imagem térmica por infravermelho, conhecida como termografia, é um recurso complementar diagnóstico sem contato e sem contraste, utilizada há décadas em diversas aplicações médicas. Porém, ainda são poucos os estudos na literatura sobre seu uso na oftalmologia. Neste trabalho os autores apresentam um caso de dacriocistite com obstrução dos pontos lacrimais onde não foi possível utilizar a técnica convencional de dacrioscistografia relatando o uso com sucesso da termografia como propedêutica complementar e discutem o método.

RESUMO Objetivo: Avaliar a relação entre a idade da intervenção cirúrgica em pacientes portadores de obstrução congênita das vias lacrimais e o sucesso obtido pelos mesmos. Métodos: Foram estudados 94 prontuários de pacientes com diagnóstico de obstrução congênita de vias lacrimais, assistidos no setor de oculoplástica do Hospital de Olhos do Paraná, no período de janeiro de 2006 a dezembro de 2012. Para diagnóstico e inclusão na pesquisa foram realizados a anamnese, exames oftalmológico geral e específico utilizando o Teste de Milder e o Teste de Obtenção de Fluoresceína na Orofaringe. Para determinar o tipo de intervenção (sondagem ou intubação) foram utilizados os critérios do serviço. Na análise estatística foram utilizados o teste de qui-quadrado e o teste t de Student e adotado o nível de significância de 5%. Resultados: Quando comparados os resultados por idade, independente do tratamento, não houve associação significativa (p=0,223) entre a taxa de melhora e a idade. Conclusão: No presente estudo não houve diferença significativa entre a intervenção cirúrgica e as faixas etárias abordadas.


ABSTRACT Objective: To evaluate the relationship between age at surgery in patients with congenital lacrimal obstruction and success achieved by them. Methods: We studied 94 medical records of patients diagnosed with congenital lacrimal obstructions, assisted at the oculoplastic sector of the Hospital de Olhos do Paraná, in the period january 2006 to december 2012. For diagnosis and inclusion in the study, the clinical history were made, overall and specific ophthalmologic examination, with the tests: Milder and Observation Test of Fluorescein in the Oropharynx. To determine the type of intervention (probing or intubation) service criteria were used. In the statistical analysis, the chi-square and Student’s t test were used. Significance level of 5 % was adopted. Results: When comparing the results by age, regardless of treatment, there was no significant association (p = 0.223) between the rate of improvement and age. Conclusions: In the present study shows, there is no significant difference between the surgical interventions within the age groups addressed.

OBJECTIVE: The bony nasolacrimal duct (BNLD) morphology as a contributory factor in primary acquired nasolacrimal duct obstruction (PANDO) is still controversial. The objectives of this study were to evaluate the morphometric differences of BNLDs in unilateral PANDO patients between PANDO and non-PANDO sides, as compared with the control group using multidetector computed tomography (CT). MATERIALS AND METHODS: Bilateral BNLDs in 39 unilateral PANDO patients and 36 control subjects were retrospectively reviewed. CT images with 0.5-mm thickness were obtained with a 64-slice scanner. The length, volume, coronal orientation type, sagittal orientation angle of BNLD, and relative lacrimal sac-BNLD angle were assessed. The entrance, minimum and distal end transverse diameters (TD) of BNLD was investigated. RESULTS: The mean minimum and distal end BNLD TDs measurements were significantly narrower in PANDO patients, both in PANDO and non-PANDO sides, as compared with the control group (p < 0.001 and p = 0.040, respectively); however, there were no significant differences between PANDO and non-PANDO sides within PANDO patients. The length, entrance TD, volume, coronal orientation type, sagittal orientation angle of BNLD, and relative lacrimal sac-BNLD angle were not significantly different between PANDO patients and control subjects, as well as between PANDO and non-PANDO sides within PANDO patients. CONCLUSION: The narrow mean minimum and distal end BNLD TD in PANDO patients, in both PANDO and non-PANDO sides, may be associated with PANDO development. The lack of difference between PANDO and non-PANDO sides within PANDO patients and some overlap between PANDO patients and control subjects suggest that narrow BNLD is not the sole factor.

ABSTRACT

PURPOSE: To identify and analyze the role of preoperative computed tomography (CT) in patients with tearing symptoms with nasolacrimal duct obstruction (NLDO). METHODS: We retrospectively reviewed the medical records and CT results on 218 patients who complained of tearing symptoms with NLDO between January 2014 and December 2014. All patients were recruited from Kim's Eye Hospital's outpatient clinic and assessed by clinical history, examination, and CT to evaluate periocular pathology and nasolacrimal drainage system. Patients with abnormal findings assessed by preoperative CT were further reviewed. RESULTS: CT was performed on 218 patients (average age, 58.2 ± 11.9 years). Of these, 196 (89.9%) had endonasal dacryocystorhinostomy, 14 (6.4%) declined surgery, and 8 (3.7%) were inoperable due to abnormal CT findings. Soft tissue opacity was the most common finding which 243 cases (85.9%) of 283 obstructed nasolacrimal duct and 89 cases (81.7%) of 109 non-obstructed nasolacrimal duct showed it. Thirty-nine (17.8%) of 218 patients showed either maxillary sinusitis or ethmoidal sinusitis and 32 (14.7%) of 218 patients presented with periocular inflammation. Other abnormal CT findings included septal deviations, previous fractures, masses, and structural abnormalities of nasal cavity. CONCLUSIONS: Preoperative CT imaging is useful in the assessment of both nasolacrimal drainage and nearby anatomical structures. This information will be helpful in planning surgical interventions and management of NLDO.

ABSTRACT

PURPOSE: To evaluate prognostic factors affecting silicone tube intubation outcomes in Asian patients with lacrimal drainage system stenosis. METHODS: A retrospective review was conducted on the medical records of 822 patients (1,118 eyes) who had undergone silicone tube intubation to treat lacrimal drainage system stenosis between January 2011 and December 2012. Patients were divided into two groups: a success group and a failure group. Success was defined as the disappearance of epiphora symptoms, normalization of tear meniscus height, and the easy passage of fluid without resistance on the postoperative syringing test. Patient and ocular parameters were compared between the success and failure groups. RESULTS: A total of 994 eyes of 727 patients were included in analyses. Patients had a mean follow-up period of 34.11 ± 18.70 weeks. Silicone tube intubation was successful in 67.2% of participants. Significant differences between the success and failure groups were found for age (p < 0.001), history of ipsilateral facial palsy (p = 0.028), follow-up period (p < 0.001), and degree of passage on the preoperative syringing test (p = 0.001). Only age (p < 0.001) and degree of passage on the preoperative syringing test (p = 0.002) remained significantly associated with silicone tube intubation success in multivariate analysis. CONCLUSIONS: Age was negatively associated with silicone tube intubation success in patients with lacrimal drainage system stenosis. The success rate was higher in patients who showed easy passage of fluid without resistance on the preoperative syringing test. These factors should be considered by surgeons planning silicone tube intubation in patients with lacrimal drainage system stenosis.

ABSTRACT

A dacriocistocele representa um saco lacrimal dilatado e é frequentemente considerada como tendo uma etiologia congênita. No entanto, dacriocistocele é uma doença rara em adultos. A característica clínica se caracteriza por um abaulamento indolor na região medial da órbita, inferior ao ligamento cantal medial. O teste de excreção lacrimal, endoscopia, tomografia computadorizada e ressonância magnética são utilizados para fazer o diagnóstico de dacriocistocele. Semelhante a pacientes pediátricos, marsupialização endoscópica do cisto nasal e a colocação de stent parece ser a terapêutica adequada. Em alguns casos o tratamento consiste na dacriocistorrinostomia externa.


The dacryocystocele represents a dilated lacrimal sac and is often considered as having a congenital etiology. However, dacryocystocele is a rare disease in adults. The clinical feature is characterized by a painless bulge in the medial region of the orbit, below the medial canthal ligament. The lacrimal excretion test, endoscopy, computerized tomography and magnetic resonance image are used to make the diagnosis of dacryocystocele. Similar to pediatric patients, endoscopic marsupialization of the cyst with nasal stenting seems to be the appropriate therapy. In some cases the treatment consists of external dacryocystorhinostomy.

ABSTRACT

A dacriocistocele representa uma rara anomalia congênita da região medial da órbita, causada pela obstrução distal (ao nível da válvula de Hasner) e proximal (ao nível da válvula de Rosenmüller) da via lacrimal, com subsequente dilatação do saco lacrimal. Recebe o nome de mucocele, quando seu conteúdo representa muco, ou amniocele, quando o seu conteúdo é preenchido por fluido amniótico. Acomete somente 0.1% das crianças, com obstrução do ducto lácrimonasal, sendo comumente unilateral e mais frequente no sexo feminino e com predisposição familiar. O diagnóstico é realizado pelas características clínicas: lesão cística tensa, abaixo do tendão cantal medial, de coloração azulacinzentada, rósea ou vermelha acompanhada por epífora desde o nascimento. No entanto podemos utilizar exames de imagem para diagnosticar esta anomalia congênita tais como: tomografia computadorizada, ressonância magnética e ultrassonografia.


The dacryocystocele represents a rare congenital anomaly in the medial region of the orbit caused by distal obstruction (Hasner valve) and proximal (valve Rosenmüller) of the lacrimal system causing dilation of the lacrimal sac. Mucocele is called when the content is mucus and amniocele when the content is filled with amniótico fluid. The incidence is only 0.1% in children with nasolacrimal duct obstruction. It is commonly unilateral and more frequent in women with familial predisposition. The diagnosis is made by clinical features: tense cystic lesion below the medial canthal tendon, blue-gray, pink or red color with epiphora since birth. However we can use image tests to diagnose this congenital anomaly such as tomography computerized, magnetic resonance and ultrasonography.

ABSTRACT

Iodoterapia representa a terapia com iodo radioativo. A radiação beta emitida pelo iodo radioativo são partículas que irão se armazenar no tecido da tireóide destruindo as células cancerígenas que ainda restaram após a cirurgia (tireoidectomia). É importante enfatizar este possível efeito deletério da terapêutica, a qual afeta a mucosa da via lacrimal, sendo o resultado do processo de inflamação e cicatrização vascular, produzindo transtornos hipovasculares, hipocelulares e hipóxicos.


Iodotherapy represents the radioactive iodine therapy. The beta radiation emitted by radioactive iodine are particles that will be stored in the thyroid tissue destroying cancer cells that were left after surgery (thyroidectomy). It is important to emphasize a possible deleterious effect of therapy which affects the mucosa of the lacrimal system which results in a process of vascular inflammation and scarring producing disorders hypovascular, hypocellular and hypoxic.

ABSTRACT

The most common abnormality of the lacrimal drainage system is congenital or acquired nasolacrimal duct obstruction. The causes of acquired nasolacrimal duct obstruction may be primary or secondary. The secondary acquired obstructions may result from infection, inflammation, neoplasm, trauma or mechanical causes. The maxillary sinus cysts usually obstruct the nasolacrimal duct mechanically. Dentigerous cysts are one of the main types of maxillary cysts. These cysts are benign odontogenic cysts which are associated with the crowns of unerupted teeth. The clinical documentations of mechanical nasolacrimal duct obstructions due to a dentigerous cyst in the maxillary sinus are very rare in literature. In this case report, we describe a dentigerous cyst with a supernumerary tooth in the maxillary sinus in an 11-year-old male child causing an obstruction to the nasolacrimal duct. The case was successfully managed surgically by Caldwell Luc approach.

ABSTRACT

Variations in the bony components of the nose are often encountered. One such variation was found in a 49-year-old male who had undergone conventional external dacryocystorhinostomy for adult onset nasolacrimal duct blockage. Intraoperatively, a thick bar of bone was seen beneath and parallel to the lacrimal sac fossa after a complete osteotomy had been made. Another osteotomy had to be fashioned in this bone to reach the nasal cavity. Postoperative 3D computed tomographic scan revealed the bone to be an anatomical variation of the uncinate process of the ethmoidal bone which was rather anteriorly placed, much thicker than usual, and attached to the nasal roof. The uncinate process is thin, curved and its anterior edge may frequently overlap some part of the lacrimal fossa. However, to our knowledge, the presence of such a large and thick uncinate process necessitating an additional large osteotomy has not been reported.

ABSTRACT

Apresentação de um caso de síndrome de Urbach-Wiethe com manifestações típicas, inclusive oculares. Paciente do sexo feminino, 15 anos, com quadro de prurido ocular relacionado à presença de lesões papuliformes em margens palpebrais (blefarose moniliforme), em associação com outras alterações sistêmicas. O diagnóstico foi confirmado por meio de biópsia cutânea e foi instituído uso de lágrimas artificiais, com alívio parcial do sintoma. O objetivo do trabalho é relatar um caso com manifestações características da doença atendido no Setor de Oftalmologia da Faculdade de Medicina de Marília.


We present a case of Urbach-Wiethe syndrome with typical findings, including ocular lesions. A 15-year-old girl was referred to our department complaining of itchy eyelid lesions (moniliform blepharosis) associated with other systemic manifestations. Diagnosis was confirmed by performing skin biopsy. Artificial tears were prescribed, with partial relief of the symptom. The objective of the present study is to describe a typical case of Urbach-Wiethe syndrome attended at the Ophthalmologic Sector of the Medical School of Marília.

ABSTRACT

Os autores realizam um estudo revisional e iconográfico das vias lacrimais através dos métodos radiológicos, sendo eles a radiografia convencional, a tomografia linear, a tomografia computadorizada e a ressonância magnética. Os métodos de imagem são fundamentais para definir diagnóstico e terapia, pois, além de demonstrarem as alterações das vias lacrimais, sugerem quais os pacientes que terão melhor prognóstico com a abordagem cirúrgica. Pelo seu custo mais baixo, menor dose de radiação, baixo índice de complicações e pela informação que pode ser obtida, recomenda-se que a dacriocistografia por tomografia linear seja o método inicial de investigação.


The authors perform a revisional and iconographic study of the lacrimal system by means of radiological methods, namely, conventional radiography, linear tomography, computed tomography and magnetic resonance imaging. Image methods are essential to define diagnosis and therapy, considering that besides demonstrating alterations of the lacrimal system, they may indicate the patients with better prognosis associated with the surgical approach. Considering the lower cost, lower radiation dose, low rate of complications, and level of information that can be obtained dacryocystography by means of linear tomography is recommended as the initial investigation method.

ABSTRACT

The diagnosis and treatment of the lacrimal passage obstruction with lacrimal endoscope was investigated and its subsidiary surgical procedures were evaluated. Ninety-three patients (109 eyes) with lacrimal passage obstruction, including presaccal canalicular obstruction (PSCO) and nasolacrimal duct obstruction (NLDO), were examined under a lacrimal endoscope, and the obstructions were treated with laser or micro-drill. All patients were followed up after the operation for 3-6 months. The difference between the laser and the micro-drill treatment was observed. During the period of follow-up, the curative rate was 82.57%. The healing rate in PSCO group and NLDO was 80.36% and 84.91% respectively (P>0.05). After treatment with the laser, the healing rate was 93.33% in the PSCO group and 66.67% in the NLDO group respectively (P<0.05). After treatment with the micro-drill, the healing rate in PSCO and NLDO groups was 65.39% and 94.28% respectively (P<0.01). The lacrimal passage obstruction can be observed and treated directly through the lacrimal endoscope. Choosing different surgical procedures in operation according to the locations of the obstruction is helpful to improve the effectiveness.

ABSTRACT

Lacrimal sac tumor is a rare tumor, for which we do not know the exact incidence. Malignant tumors account for 70% of all cases. Epithelial tumors are the most common pathological type and squamous cell carcinoma is the most common of these, a transitional cell carcinoma is rare and has a poor prognosis. The authors report a rare case of transitional cell carcinoma of the lacrimal sac. A 57-year-old woman had tearing on her right eye. The pre-operative diagnosis was possible nasolacrimal duct obstruction with signs of dacryocele. Intraoperatively the authors found a lacrimal mass and a frozen section showed squamous papilloma with dysplasia. The tumor was completely removed and DCR was done. The subsequent pathological report of the right lacrimal sac was papillary transitional cell carcinoma, so medial maxillectomy and resection of the medial inferior orbit with ethmoidectomy were performed. She received radiation and has been tumor-free for 2 years.

ABSTRACT

A Dacriocistorrinostomia (DCR) consiste em criar uma via de drenagem lacrimal para a cavidade nasal, tendo como objetivo restabelecer a drenagem permanente deste sistema excretor previamente obstruído. OBJETIVO: Relatar nossos resultados com a técnica de DCR endoscópica, expondo vantagens e desvantagens em relação à técnica externa. FORMA DE ESTUDO: Coorte histórica. MATERIAL E MÉTODO: Foram analisadas retrospectivamente trinta e duas dacriocistorrinostomias realizadas pela Disciplina de Otorrinolaringologia de março de 2002 a janeiro de 2004 em pacientes com obstrução pós-saco lacrimal comprovada por dacriocistografia (DCG). Todos foram submetidos à sondagem com sonda de Crawford. RESULTADOS: Dos vinte e dois pacientes analisados, a cirurgia foi bilateral em dez totalizando trinta e dois procedimentos, sendo vinte e nove cirurgias primárias e três casos revisionais de insucessos com DCR externa. Nossa taxa de sucesso foi de 79,12 por cento. CONCLUSÕES: A DCR endoscópica mostrou ser uma técnica segura, com baixa morbidade, além de evitar cicatrizes faciais e manter o mecanismo de bomba lacrimal, com resultados semelhantes à DCR externa.
